Dr. Sharma is accepting new patients by referral only. Please have them fax a referral to us with any medical records or Radiology (MRI, CT, X-ray) that are medically pertinent to the treatment of your pain. Our fax and other contact information can be viewed on the contacts tab of this website. If you have any questions please feel free to call us and we will do our best to answer any questions.
When can I be scheduled?
After we receive your referral and records the doctor will personally review them and set us a day to schedule you.
What to bring to initial appointment?
- Valid Florida I.D
- List of medications you are taking
- Any MRI, CT, X-ray discs you have
- Any other medical records you wish to discuss with the doctor
